Agreement, Waiver, and Release Form

to participate in the Junior Firefighter Camp. In consideration of the above described program, I, on behalf of my minor child, and myself hereby agree to release and hold the Town of Fort Erie, its officers, agents, employees and volunteers harmless from any and all liability for any injury or loss suffered by my minor child or myself, arising from or in any way connected to this program.

Photo Release

I give permission to the Town of Fort Erie, the Fort Erie Fire Department to photograph me or my child(ren) participating in the programs for use in publicity or publications for the Town of Fort Erie, the Fort Erie Fire Department and will not seek compensation for such.
